One of the PCs is bitten by a cranium rat. A DC 15 Fortitude save must be made to avoid contracting lycanthropy and becoming an afflicted cranium wererat.
If failed, the PC has very vivid and nonsensical dreams over the next several nights, and begins to hear faint whispers and chittering noises when cranium rats are nearby. After one week, the subject's neural pathways are completely reshaped, opening him constantly to the thoughts of the cranium rat collective.
From here, it shouldn't be hard to find something for Ilsensine's newest unwilling servant to do....
